This is a nice simple detailed diagram of our (male) sexual and urinary apparatus. For effective massage, milking or exercise results it is not necessary to learn all the parts inside of the gland.

Although the tissue change is different for BPH (growth) and inflammation (swelling), the size increase can cause the same problems: reduced urine flow (slow stream) and frequent need to pass small amounts of urine.
BPH itself is rarely painful because the growth is slow. It can, however, progress to the point of actually cutting off Urine flow. That is a painful medical emergency and should be treated as such.
Inflammation from prostatitis, in my experience, has not completely cut the urine flow off (though it possibly could). That again would be a medical emergency that should be handled immediately.
Swelling from inflammation, however does often happen rather quickly. This is what causes most of the intense pain associated with prostatitis. You can also have the pain from irritation of internal toxins.
But, the rapid swelling tries to stretch the outer covering (fascia) of the gland beyond it's ability to stretch.Once the gland shrinks again, the pain subsides and goes away.
Just like when you get shin splints. It's the same situation. And both can be extremely painful.

"Digitial" simply means: with a digit (a finger).
He is reaching in with a finger to feel your prostate. He feels for:
A basic diagnosis of your prostate condition can often be made from just these findings.
This is also what a prostate masssage or a prostate milking with another persons finger would look like.
You'll notice that the massage, the milking, and the digital rectal exam all look the same.
The reason for this is that the most direct contact can be made with the prostate gland this way.

A healthy gland is about the size of a walnut. You will be able to feel it through your stomach wall if there is not too much fat there.
If the gland is inflamed or sore, you will notice it right away when you touch it this way.
